Faculty Availability and Contact Information (DUE UPON LOGIN)

Upon login to the Online Faculty Center, please be sure to let us know if you will be available to participate. Please also be sure to update your complete contact information and provide us contact information for your assistant if applicable.

Topic Submissions

In order to build the most up to date and comprehensive program possible, the course directors invite you to suggest topics that you would like to present at CCC 2010. Topics should address specific educational needs that you perceive in the cardiovascular community and suggestions should be accompanied by a brief description and one learning objective. Remember that lectures are limited to 10-15 minutes depending on the session.

AACN Biographical Data Form (DUE UPON LOGIN)

We will be applying to the American Association of Critical-Care Nurses (AACN) for Continuing Education credits for nurses. As part of the application process a Biographical Data form must be completed for each speaker. We ask that you please complete this form as part of your confirmation to participate in CCCM 2010. We thank you very much for your assistance.

Faculty W-9 Form (DUE FEBRUARY 13, 2010)

U.S. faculty are required to complete a W-9 Form in order to receive their honorarium check. (International Facutly may click accept to by pass this step.) The W-9 Form should be completed and provided to Complete Conference Management as soon as is possible after logging in.
